Podcast: Dementia Care Family Support Program

Recently, I joined the team from Plans4Care to discuss our Dementia Care Family Support Program on All Home Care Matters, a podcast and YouTube show.

Through the Dementia Care Family Support program, Plans4Care, Inc. and Caregiving Years Training Academy created a unique 5-session program that combines the lived experience of former caregivers and professionals trained in supporting caregivers with Plans4Care’s digital Caregiver Playbook. Family caregivers work with Certified Caregiving Consultants over five sessions to address up to three care challenges, using personalized care strategies generated by the Caregiver Playbook based on the specific functional ability of the person with dementia

During our podcast discussion, we shared what we learned about using technology during coaching sessions, the common care challenges we addressed and the insights we gained from tracking our coaching sessions. We also talked about what we learned about how to best support dementia family caregivers to help ease their stress and worries.

About Dr. Laura N. Gitlin:

Dr. Laura N. Gitlin is the Co-founder and Chief Scientific Officer at Plans4Care Inc.. She is an intervention scientist with over 40 years of experience in dementia care and research. Dr. Gitlin currently serves as a multiple Principal Investigator on several NIH-funded grants, where she leads the development and testing of innovative interventions to support people living with dementia and their caregivers.
